I have no qc issues with my ironhide at all, sucks that I didn't get that accessory.

The original was released not so long ago and TakaraTomy is already re-releasing Ironhide with a new accessory In fact, I have just received mine...

When you said "your shoulders were installed in the correct orientation" do you mean I see no screw holes facing the front? In this case mine also does not have the QC issue. Ditto w/ my MP Ratchet.

For those who wants that accessory, its asia exclusive. So if u order from japan, u wont get it. Usually, it cost more to get it from outside of japan.

No. Look at the top of the shoulders an compare with MP Ratchet. The ratchet joint on Ironhide is exposed on most of them due to assembly error.
