Custom Contest Entry Form

For eveyone who will be entering the contest, it would really help out with the flow of things on Saturday morning if you could print, fill out and bring the following form with you. The Entry Number at the bottom can be left blank, it will be assigned when you sign in.

This year we've added an optional line for an email address. Last year we had a lot of the public asking if there was some way to contact the artists about doing comissions. If this is something that interests you, be sure to add your email address.

Thanks to all who will be competing this year. Everyone is looking forward to seeing some really stiff competition.

Some forms will be available at the table for those who may not have read this thread or had trouble printing. That being said it would be a big help if those who are able to print ahead of time would bring the form with them. Cheers!

Nto that I know of but people have expressed interest in buying certain entries so we decided to put an optional contact info e-mail so those interested can contact the customizer themselves.

i was offered but i declined last year(after i deciphered that the japanese guy was first asking me where i "bought" my project),but once i explained it was one of a kind,i then made the mistake of not noticing wifey right behind me when i turned down 4 grand cash.......oops up side the head


back on topic,i may have to do it the old school way and fill it out at the con as my printer's on the fritz
