Primus was at the centre of Cybertron, created the Transformers, and stored some of his essence into the Creation Matrix.
Unicron kills Primus and O.Prime uses the Matrix to destroy Unicron. Afterwards, the Matrix is kindof considered spent.

Cybertron begins falling apart after Primus' death, but a prophecy of the Last Autobot has Hi-Q (merged mentally with Prime) not abandon the planet and he finds the Last Autobot (with some strange help).

Primus left the Last Autobot to guard over after he would die. So L.A. recreates Prime and Hi-Q into 1 new Optimus Prime.
